Tenancy

Triniti Care enables supported living accommodation where individuals hold their own tenancies with reliable housing stock partners, typically within HMO-licensed premises.

We have agreements with estate owners to ensure that certain features of the property remain adjustable to the potential severity of an individual’s needs.

• Individual tenancies with housing-related support • Flexible day-time support with night-time cover (sleep-in or waking night) • Shared communal areas alongside breakout/quiet spaces • Staffing levels determined by risk assessment and support need • A focus on tenancy sustainment and relapse prevention

Tailored care plans

Prevention is better than cure.

Our model prioritises prevention-led support focused on early intervention, relapse management, and crisis avoidance to sustain stability and reduce admissions.
Structured engagement and personalised plans enabling meaningful access to community, education, and vocational pathways.

Delivered in line with Care Act, Mental Capacity Act, and CQC Fundamental Standards, supporting independence, reducing delayed discharges, and enabling safe step-down from inpatient and residential care.
